How slab moisture mitigation gets specified
Moisture mitigation is specified off test data rather than opinion. Relative humidity probes placed in the slab, per the widely used in-situ method, report the moisture condition at depth, and flooring and coating manufacturers publish a maximum their product tolerates.
Above that threshold, a vapor barrier goes down before the finish system. Below it, no barrier is needed and the documentation showing why is worth keeping. Either way the test costs a fraction of what a failed floor costs to remove and reinstall.
